Reports and Evaluations
In an effort to both evaluate the Foundation's effectiveness and also to maintain a high level of fiscal responsibility, the Foundation will conduct a post grant evaluation for every charitable distribution. As part of this evaluation process, every grantee organization will be required to submit a completed Common Grant Report by December 31st of the same year in which they received funding.
The acceptable Common Grant Report Form, may be downloaded from this web site. The foundation requires that the responsible officer in each recipient organization submit this written report on what has been accomplished with the funds granted. The funds granted may only be expended for the purposes indicated; funds not so spent must be returned to the foundation unless other arrangements have been approved by the foundation.
The foundation will greatly appreciate the grantee's cooperation in this effort.
